All Cosmetic Procedures must be paid for prior to treatment. We accept Cash, Personal Checks, and Mastercard/Visa/American Express Credit Cards.
Insurance Information
Please bring your insurance card to your appointment, along with your referral information. You may also have your primary care physician fax your referral to our office at (312) 695-4303.

The Patient Services Center works to make sure your experience with the NMFF is a good one. If you or your family has a question or concern about your stay at our hospital, please let us know. We suggest you first discuss your concerns with your nurse, the department manager and your doctor. If your concern remains unresolved, the Patient Services Center staff is available to help reach a resolution or provide more information. If you are using the CTA, take the...
· Dan Ryan-Howard Red Line to Chicago Avenue. Exit the station, and catch the #66 CTA
bus east to Huron Street and Fairbanks Court.
· Douglas Park-54 Cermak Blue Line to Washington Street. Transfer to the Red Line, and
go north to Chicago Avenue. Exit the station, and catch the #66 CTA bus east to
Huron Street and Fairbanks Court.
· Forest Park Blue Line to Washington Street. Transfer to the Red Line, and go north to
Chicago Avenue. Exit the station, and catch the #66 CTA bus east to Huron Street
and Fairbanks Court.
· Ravenswood Brown Line to Belmont or Fullerton Avenue. Transfer to the Red Line, and
go south to Chicago Avenue. Exit the station, and catch the #66 CTA bus east to

· Midway Orange Line to State and Lake Streets. Transfer to the Red Line, and go north

· Rock Island trains to LaSalle Street. Exit the station, walk north one block to Jackson
and Financial. Take the CTA 151 bus northbound to Huron Street. Walk east to Fairbanks
Court.
· Union Station trains to Adams Street. Exit the station, walk south to Canal between
Jackson and Adams. Take the CTA 151 bus northbound to Huron Street. Walk east to
Fairbanks Court.
· Northwestern Station trains to Madison Street. Exit the station, walk south 2 blocks
to Canal between Jackson and Adams. Take the CTA 151 bus northbound to Huron
Street. Walk east to Fairbanks Court.
· Electric Line trains to Randolph Street. Exit the station, walk west to Michigan Avenue
and take a northbound bus to Huron Street. Walk east to Fairbanks Court.
If you are driving...
· From the south take the Dan Ryan Expressway (94W) north to Lake Shore Drive. Go
north on Lake Shore Drive to Chicago Avenue. Turn left on Chicago Avenue to Fairbanks
Court. Turn left on Fairbanks Court to Huron Street. The building will be on the southwest
corner.
· From the north take the Edens Expressway (94E) south to the Kennedy Expressway
(90E). Take the Kennedy Expressway (90E) south to Ohio Street. Go east on Ohio Street
to St. Clair Street (one block east of Michigan Avenue). Turn left on St. Clair Street to
Erie Street. The building will be on the northeast corner.
· From the northwest take the Kennedy Expressway (90E) south to Ohio Street. Go
east on Ohio Street to St. Clair Street (one block east of Michigan Avenue). Turn left
on St. Clair Street to Erie Street. The building will be on the northeast corner.
· From the west take the Eisenhower Expressway (290) east to Columbus Drive (one
block east of Michigan Avenue). Turn left on Columbus Drive (becomes Fairbanks Court)
to Huron Street. The building will be on the southwest corner.
· From the southwest take the Stevenson Expressway (55) east to Lake Shore Drive.
Go north on Lake Shore Drive to Chicago Avenue. Turn left on Chicago Avenue to
Fairbanks Court. Turn left on Fairbanks Court to Huron Street. The building will be on the
southwest corner.
Parking
Convenient parking is available in the parking lot at the corner of Huron and St. Clair Streets. You may enter this lot from either Huron or Superior Street. Discount parking is available to patients. Please ask your physician's office staff for details.
